License Verification can now be done directly on our website. All licensees listed in the Optometrist Directory are current and in good standing. Any disciplinary actions will be marked by an asterisk, and would be listed on the Disciplinary Actions page. The website is updated daily so it is always up to date. Under the Open records Act, various lists of active licensees can be provided by the Board. Information included on these lists may include name, office addresses and phone numbers, license numbers, license status, dates of license, and renewal dates. To obtain a list, contact the Executive Director of the Board. Most lists are emailed in excel format.
